For many couples navigating fertility challenges in Calgary and across Canada, IVF feels like the inevitable destination. The referral comes, the consultation is booked, the credit card is readied. And in the rush of hope and urgency that surrounds the decision, one critical question almost never gets asked:
Is my body actually ready?
Not ready in the sense of being willing — of course you’re willing. Ready in the sense of being physiologically optimized — so that the significant financial and emotional investment you’re making has the strongest possible biological foundation beneath it.
This post is not about whether you should do IVF. That is a deeply personal decision, and one I respect entirely. This post is about the 90-day window before your retrieval — what it is, why it matters, and what targeted Naturopathic preparation can do within it that your clinic’s protocol alone cannot.
What IVF Does and Doesn’t Do
IVF is a remarkable technology. It can overcome blocked tubes, severe male factor infertility, and a range of other structural barriers to conception that no amount of natural optimization can address. For the couples who need it, it is genuinely life-changing.
But it is important to understand what IVF does and does not do.
IVF works with the biological material your body provides — the eggs retrieved from your ovaries, the sperm collected from your partner, the uterine environment in which the embryo will be asked to implant. It does not improve the underlying quality of those materials. It does not correct nutritional deficiencies that impair egg maturation. It does not fix sperm DNA damage. It does not optimize your uterine inflammatory environment. It does not address subclinical thyroid dysfunction that increases miscarriage risk even in IVF pregnancies.
Those variables — the quality of what your clinic works with — are shaped in the months before retrieval. And research increasingly shows that they matter enormously for IVF outcomes.
The 90-Day Window — Why It Matters More Than Anything Else
Eggs take approximately 90 days to mature from their earliest developmental stage (primordial follicle) to the point of retrieval. Sperm takes 72 days to fully develop. This means that the nutritional environment, the hormonal environment, the inflammatory environment, and the oxidative stress load your body is carrying in the three months before your retrieval directly determines the biological quality of the eggs and sperm your clinic will work with.
This is not a small variable. It is arguably the most significant modifiable factor in IVF success — and it is almost entirely within your control.
Most couples begin thinking seriously about IVF preparation after their retrieval date is already set, leaving weeks rather than months to work with. Starting earlier — ideally 3 months before retrieval — gives you the full benefit of this biological window.
The Six Domains of Naturopathic IVF Preparation
In my Naturopathic fertility practice in Calgary, IVF preparation is built around six core domains — each targeted at a specific aspect of the biological picture your clinic will work with.
Egg Quality Optimization
The mitochondria inside each egg are responsible for the energy production that powers fertilization, the early cell divisions of embryo development, and implantation. Mitochondrial function is the primary determinant of egg quality — and it is directly supported by targeted nutrition.
CoQ10 is the most evidence-supported supplement for egg quality improvement. Multiple clinical trials have demonstrated improvements in ovarian response, egg quality, and clinical pregnancy rates with CoQ10 supplementation at doses of 400–600mg daily — with the greatest benefit when begun 90 days before retrieval.
Vitamin D at therapeutic doses to achieve serum levels of greater than 125 nmol/L is independently associated with improved IVF clinical pregnancy rates. In Calgary, where vitamin D insufficiency through winter months is near-universal, this is one of the most consistently relevant pre-IVF interventions.
DHEA (at doses of 25–75mg daily under supervision) shows meaningful benefits for poor responders — women with diminished ovarian reserve — in improving ovarian response to stimulation. Should be used under medical supervision with appropriate testing.
Sperm Health and DNA Fragmentation
A comprehensive male assessment is essential before IVF — not because IVF can work around all sperm quality issues, but because sperm DNA integrity directly affects embryo quality, development, and miscarriage risk in ways that intracytoplasmic sperm injection (ICSI) cannot correct.
Sperm DNA fragmentation testing, if not already done, should be a priority. High fragmentation (DFI above 15–25%) is addressable with targeted antioxidant therapy — but requires 90 days of treatment before retrieval to affect the sperm in current development.
The core antioxidant protocol for sperm DNA fragmentation includes: vitamin C and E at antioxidant doses, CoQ10, zinc, N-acetylcysteine, and selenium — alongside lifestyle modifications targeting the oxidative stressors (heat exposure, alcohol, smoking, poor sleep) that drive fragmentation.
Thyroid Optimization
Subclinical hypothyroidism — TSH above 2.5 — is associated with significantly reduced IVF success rates and meaningfully increased miscarriage risk, even in IVF pregnancies. The European Society of Human Reproduction and Embryology (ESHRE) guidelines recommend optimizing TSH below 2.5 before IVF — a recommendation that is not always acted upon in clinical practice.
In my experience, thyroid optimization is one of the most impactful and most frequently overlooked components of IVF preparation. A full thyroid panel including Free T3, Free T4, and thyroid antibodies — not just TSH — provides the complete picture needed to intervene appropriately.
Uterine Lining and Implantation Environment
A receptive uterine lining requires adequate nutritional support (iron, vitamin E, L-arginine, omega-3 fatty acids), a healthy uterine microbiome influenced by gut health, and a manageable systemic inflammatory burden. Each of these is addressable in the months before transfer.
The emerging research on the uterine microbiome is particularly relevant for couples with recurrent implantation failure — a disrupted uterine microbiome is increasingly being identified as a contributing factor, and gut health restoration is one of the most promising avenues for improving uterine receptivity.
Methylation and Nutrient Status
MTHFR gene variants affect the body’s ability to process folate and B12 — with direct implications for egg quality, embryo development, and early pregnancy support. In carriers (40–60% of the population), methylated forms of folate and B12 are essential rather than optional. Folic acid alone — even at prenatal vitamin doses — is insufficient.
A comprehensive nutrient panel before IVF should include folate, B12, vitamin D, iron studies, CoQ10, zinc, and magnesium — identifying deficiencies that are both clinically significant and readily correctable before your cycle begins.
Stress Response and HPA Axis
Cortisol dysregulation measurably reduces ovarian response to gonadotropin stimulation — the hormone injections used to stimulate egg production in IVF. A body under chronic physiological stress does not respond to stimulation as well as one with a regulated cortisol rhythm.
HPA axis support in the 60–90 days before retrieval — through adaptogenic botanicals, targeted nutrient support, sleep optimization, and evidence-based nervous system regulation practices — is one of the highest-leverage pre-IVF interventions available. And it is almost never discussed by fertility clinics.
How This Works With Your Clinic
Naturopathic IVF preparation is designed to work alongside your fertility clinic — not instead of it. I work in open communication with my patients’ reproductive endocrinologists in Calgary and across Alberta. I share supplement protocols before stimulation begins — ensuring nothing interferes with the medical protocol — and adjust the Naturopathic work based on clinic feedback.
Many reproductive endocrinologists are increasingly supportive of evidence-based Naturopathic co-management. The goal is always the same: the best possible outcome for their patient.
